Responsibilities: * Provides for activities of daily living by assisting with serving meals, feeding patients as necessary; ambulating, dressing, turning, and positioning patients; providing fresh water and nourishment between meals.

* Demonstrates ability of providing assistance with resident grooming and hygiene needs.

This would include: complete/partial baths, oral care, dentures, Foley care, hair/nail care, shaving, and bed making.

* Assists in the toileting needs of the residents, records outputs accurately.

Empties drainage collection devices, completes documentation of BM and participates in toileting scheduling, as necessary.

* Answers call lights promptly; responds promptly to resident inquiries, reports any abnormal findings or complaints to licensed nurse.

* Obtains vital signs, weights and intake and output at the direction of the licensed nurse.

* Participate in resident programming, as directed.

* Assists the nurse with various duties as directed.

* Ensure accurate reporting of all injury/accidents in the facility to the licensed nurse timely.

* Ensure all documentation is completed accurately.

* Communicate effectively with licensed nurse regarding resident care issues.

* Maintain all aspects of confidentiality relating to the residents and any policies or procedures at the community.

* Provide emotional support to residents, as needed.

* Maintain an acceptable attendance record to provide continuity of service to our residents.
