PrimaryOne Health is currently seeking an HFA (Healthy Families of America) Home Visit Program Manager for our Columbus health centers.

Essential Job Responsibilities: * Assists in recruitment and enrollment of women and their families eligible for the HFA program * Works as a family support specialists as need be and ensures continuous and comprehensive care for patients on caseload in adherence to HFA and Growing Great Kids curriculum.

* Assists in the selection of staff, participates in the orientation and training, conducts record reviews, assists in or maintains the data collection system, implements a quality assurance plan, and monitors the performance of Family Support Specialists.

* Helps staff maintain perspective, evaluate their own performance, increase personal and professional development, learn and practice new strategies to effectively work with families, and develop reflective capacity, and ultimately enhance the quality of services families receive.

* Acts as a liaison with other agencies and monitor contracts as well as ongoing program development.

* Able to provide and supervise Home Visits as needed to support onboarding new staff and support the program's caseload capacity.

* Works collaboratively with the health care team including the providers, pharmacists, dieticians, behavioral health specialist, other nursing staff and medical assistants.

* Responsible for adherence to policies, procedures, guideline and standards of HFA, Ohio Department of Health Help Me Grow, and PrimaryOne Health.

* Completes documentation of all professional activities, contacts and observations promptly and appropriately.

* Attends internal and community meetings as assigned and participates in Quality Improvement activities.

* Regular travel required.

* Attendance, promptness, professionalism, the ability to pay attention to detail, cooperativeness with co-workers and supervisors, and politeness to customers, vendors, and patients.

* Stays informed of current health care developments within the field of maternal/child health, public health, and home visitation * Other duties or special projects as assigned.

Successful candidates must have a bachelor's degree in human services or fields related to working with children and families with 3 years of relevant experience, or less than a bachelor's degree but with 3-5 years of commensurate HFA experience 1-2 years' recent experience in a maternal/child health, public health, home visiting or equivalent combinations of education/experience preferred.

Supervision experience with reflective practice preferred.

Experience in providing trauma-informed family support services preferred.

Experience in supporting culturally diverse communities/families preferred.
