They are responsible for managing the overall health and well-being of patients, ensuring personalized care plans, and collaborating with interdisciplinary teams to promote optimal outcomes.\n\nKey Responsibilities:\n\nProvide direct nursing care to residents in accordance with individualized care plans.\nAdminister medications, perform treatments, and monitor patients’ progress, ensuring safety and adherence to established protocols.\nAssess, plan, implement, and evaluate nursing care for long-term care residents.\nCollaborate with physicians, therapists, and other healthcare professionals to develop and adjust patient care plans.\nMonitor and document residents’ health status, including vital signs, medication administration, and overall progress.\nEducate residents and their families on health conditions, care needs, and treatment options.\nRespond to and manage acute and chronic conditions, ensuring timely interventions and care coordination.\nProvide emotional support and counseling to residents and their families, promoting a positive living environment.\nEnsure compliance with all federal, state, and facility regulations related to long-term care.\nSupervise and delegate tasks to licensed practical nurses (LPNs) and nursing assistants (CNAs).\nParticipate in quality improvement initiatives, staff training, and continuing education.\nMaintain accurate patient records and documentation, adhering to established clinical standards and protocols.\nAddress concerns related to infection control, safety, and preventative care to minimize risks to residents.\n\n\nQualifications:\n\nEducation: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.\nLicensure:\n\nValid Registered Nurse (RN) license in the state of practice.\n\n\nCertification:\n\nBasic Life Support (BLS) certification required.\nAdvanced certifications such as ACLS or Geriatric Nursing (e.g., CGRN) preferred.\n\n\nExperience: Previous experience in long-term care, gerontology, or related setting\n Wellspring Nurse Source Job ID #34251787.
