Health Guide (NP / PA) - Psych Primary Care

A healthcare organization in Ohio is actively seeking a compassionate and dedicated Nurse Practitioner / Physician Assistant (NP / PA) to join their staff as their new Health Guide.

In this role, the Health Guide (NP / PA) will assess both behavioral and physical health conditions through a thorough history and physical examinationResponsibilitiesCapturing conditions not yet diagnosed, including by ordering additional studies as indicatedOrganizing the information in a manner that supports successful disability applicationsConducting visits in the field or in an individual's home when neededMaking individualized recommendations and/or referrals for programs/careTraining and advising Guides and Community Resources Guides on clinical issues and treatmentsSupporting Guide Team on care coordinationLeading care managementPerforming medication reconciliation post-discharge and as neededCommunicating and coordinating with individuals' care providers, primarily at CMHCs, FQHCs, and hospitals Qualifications:Active OH Nurse Practitioner and/or Physician Assistant2+ years clinical care experience as a Nurse Practitioner and/or Physician AssistantExperience working with and managing a population with challenges such as behavioral health and/or SUD conditionsExperience developing and delivering educational programs to nurses and affiliated medical staffCare management and coordination experienceIntermediate computer skills; familiarity with Microsoft Office Suite Desired Skills: Primary Care experience
