The City of Topekas Housing Services Division is seeking a dedicated individual to join its team as a KDADS Boundary Spanner Program Coordinator.

This position is a partnership with Kansas Department for Aging and Disability Services (KDADS) and works to connect individuals with a behavioral health impairment to coordinated entry services.

The Boundary Spanner will receive referrals from KDADS and other state and community agencies and ensures that individuals exiting publicly funded institutions are given informed choices about community housing programs and support options.

Primary duties include: * Work as the Coordinated Entry Liaison with the Topeka/Shawnee County Continuum of Care (CoC), KDADS, the States Certified Community Behavioral Health Clinics (CCBHC) and Community Mental Health Clinics (CMHC) provider agencies.

* This includes identifying barriers and working on policy changes to ensure access to HUDs Coordinated Entry System and HUD Access points for consumers who meet eligibility requirements.

* Provide support services and interventions for qualified individuals.

* Expand the current remote VI-SPDAT screening program to target mental health consumers who meet requirements.

* Work directly with mental health consumers by completing VI-SPDATS and referrals to the Coordinated Entry Process; ensure consumers are connected to the CoCs by-name list.

* Collect and share data as needed to participate in and improve overall service delivery.

The ideal candidate: * Has knowledge of counseling principles, crisis intervention and mental health resources.

* Is relationship-driven, with the ability to interact effectively with consumers, City staff, elected officials and agency partners in a variety of settings.

* Is highly organized, with the ability to plan, coordinate and prioritize multiple tasks.

* Has the ability to work both independently and collaboratively.

Minimum qualifications: * Bachelors degree from an accredited college or university in social work or human services with one to three years of experience in these fields OR * Without a bachelors degree, a high school diploma or its equivalent is required with three or more years of experience in social work or human services.

* Must possess and maintain a valid Kansas drivers license and must be insurable within the standards set by the City of Topekas insurance carrier.
