The Manager of Quality and Population Health (Director) is responsible for the management of the medical clinic operations of SBHF, as well as providing a broad range of clinical and administrative assistance to patients.

This role, along with the President & CEO, Physicians, Director of Operations, Registered Dietitians, Health Educators, Supervisors, and supporting staff help support the Patient Centered Health Home model of care.

The Manager of Quality and Population Health helps plan and coordinate the development and implementation of San Benito Health Foundation (SBHF) population health initiatives, including, but not limited to: UDS, Clinical Measure, HEDIS, SBHF Quality Measures, and other program quality measures.

This position will also mentor Patient Care Specialists and other staff.

This role serves as the leader of strategic programs designed to improve clinical outcomes through coordination of clinical operations, information systems, administrative departments, physician leadership, and physician practices.

Determines direction, goals, and objectives of assigned programs and links to appropriate hospitals, physician.

Duties and Responsibilities: * Accountable for the management and oversight of the population health management of the clinic.

* Provides reports as required by Federal, state and local laws and regulations * Presents updates on special project to the Board of Directors * Serves as the coordinator of strategic programs designed to improve clinical outcomes through coordination of clinical information systems, administrative departments, provider leaders and provider practice.

* Manages day-to-day operations, assuring that all services are provided according to standards and protocols * Conducts routine monitoring to identify and solve problems to ensure correct operation of the medical clinic * Trains and monitors employees of the medical clinic * Employee management, problem solving, work flow development and collaboration with lead patient care specialist supervisor.

* Provides assistance to providers by being chair side next to the provider and monitoring quality of service.

* Assists in the development of long and short-term plans and objectives for the SBHF * Works with the Executive Director to develop and implement a strategic plan consistent with the purposes and objectives of the organization * Responsible for gathering data and proposing strategies to achieve measurable improvements in ambulatory clinical quality and population health management that provide value to the clinical team resulting in a strong business case for improved clinical outcomes that meet the requirements of regulatory agencies * Manages staff and facilities of the medical clinic, ensuring compliance with all rules and regulations applicable to a Federally Qualified Health Center * Performs other duties as assigned.

Knowledge Skills & Ability: * Bachelor's in Health Education, Public Health, or related field.

* Must possess 5 years of managerial experience in addition to 3 -5 years of related professional experience that includes medical services to diverse populations.

* Possesses a full understanding of the provision of primary health care in outpatient settings, as well as the operations of a and medical facility * Full knowledge of the Federal, state and local rules and regulations governing operations and management of a Federal Qualified Health Center * At least five years of work experience in a management role in health care with progressively increasing responsibilities.

* Possesses a full understanding of the needs of the low-income, seniors and ethnic minorities, typically acquired in at least two years of experience working with non-profit organizations or community groups.

* Possesses high level of verbal communication skills, including the ability to demonstrate attention to patients' and staff needs and respond accordingly * Must possess: strong leadership, organizational, team building, and personnel development skills, excellent written and verbal communication skills.

* Must have the ability to multi-task.

* Must be able to conduct community needs assessments, focus group facilitation, and program evaluation.

Must have excellent computer skills and experience with MS Windows.

* Bilingual in Spanish/English and knowledge of the Hispanic culture * Must possess a valid driver's license.

* May be required to work unusual hours or shifts, and may be required to change shifts with little notice.
