The Clinical Pharmacy Supervisor Pediatrics and Neonates will be part of a pharmacy team that cares for pediatric patients in the general pediatric unit, and the Neonatal Intensive Care Unit (NICU).
Duties include:
Practice with other members of the patient care team and focus on the needs of patients.
Regularly make rounds as a member of the multidisciplinary patient care team to optimize pharmacotherapeutic managements of patients.
Prospectively evaluate and document all drug therapy for appropriate indications, dosage, drug interactions, and drug allergies; monitors the patients' pharmacotherapeutic regimen for effectiveness and ADEs; and intervenes as needed.
Identify ADEs and assists in their management and prevention, and develops process improvements to reduce drug errors and preventable ADEs.
Provide drug information to the patient care team.
Provide drug therapy related education to healthcare team.
Provide didactic lectures to health professional residents and students in pharmacology and therapeutics, where applicable.
Perform drug utilization review.

Requirements
Qualifications/Required Education/Experience/Specialized Skills
PharmD required, Post graduate year one residency training is required, Post graduate year two residency training in Pediatrics/Neonates is preferred; OR minimum of 3 years of clinical experience in Pediatrics/Neonates
New York State Pharmacist License required
Board Certification in Pharmacotherapy preferred
