The Home Health Aide (HHA) & Rehabilitation Coordinator is responsible for facilitating home health aide and rehabilitation services (e.g., physical therapy and occupational therapy) and ensuring strong communication between Innovive staff, contract agencies, and patients regarding HHA and therapy services. This role supports daily operations and helps manage scheduling, communication, and documentation to ensure high-quality patient care is provided by therapists and home health aides.
WHAT YOU'LL DO:
• Monitor therapy and HHA referrals and follow up regularly to ensure patient needs are addressed
• Address and resolve scheduling or care coordination issues as they arise
• Communicate all service requests and changes to contract agencies and clinicians
• Confirm scheduled HHA and therapy services with Innovive clinical staff and contract agencies
• Schedule home health aide (HHA) introductions/supervisions and therapy evaluations in a timely fashion
• Track and maintain accurate schedules for therapy and HHA visits
• Verify therapy and HHA visits are completed daily, including confirming therapy visits and uploading therapy notes from external EMRs
• Notify clinical staff of missed visits or service refusals by patients or caregivers
• Serve as a liaison between Innovive staff, contract agencies, and patients to ensure care coordination for therapy and HHA services
• Build and maintain positive, ongoing communication with contract agencies and clinicians
• Monitor clinician and patient reports for trends or concerns
• Report patient complaints, grievances, and service-related concerns to the Manager and Director
• Assist the Clinical Manager and Director with daily department operations
• Attend orientations and meetings regarding HHA and therapy services
• Participate in performance improvement and staff competency programs
• Adhere to HIPAA, privacy regulations, and agency compliance programs
• Follow all company policies and procedures
• Perform other duties related to the HHA and therapy departments as assigned by the Manager or Director
WHA YOU HAVE:
• Associate's degree or equivalent in work related experience
• Minimum of 1 years' experience in an office setting, healthcare industry preferred
• Advanced proficiency in Windows environment, with emphasis on Excel, Outlook, and Word
• Ability to work in a fast-paced environment
PREFERRED EDUCATION AND EXPERIENCE:
• Bachelor's degree preferred
• 2 years' experience in a scheduling position
PHYSICAL DEMANDS AND OFFICE ENVIRONMENT:
• Occasionally required to stand.

• Frequently required to sit.
• Continually required to use hands and fingers.
• Occasionally required climb, balance, bend, stoop, kneel or crawl.
• Occasionally required to lift/push light weights (less than 25 pounds).
TRAVEL REQUIREMENTS:
• Ability to travel locally by car to office(s), events, meetings, etc., as needed (approximately 5% or less of the time).
