Transplant Clinical Pharmacist - Solid Organ Specialty

Core Responsibilities:

Lead pharmaceutical care throughout the transplant continuum, from initial patient selection to long-term follow-up

Optimize immunosuppressive medication regimens and manage complex comorbidities

Provide comprehensive medication education to patients and families

Participate in multidisciplinary rounds and clinic visits

Develop and implement treatment protocols for allograft rejection

Precept pharmacy students and residents in transplant specialty rotations

Conduct clinical research and quality improvement initiativesRequired Qualifications:

PharmD from ACPE-accredited College of Pharmacy

Virginia pharmacist license or eligibility for licensure

Completion of ASHP-accredited PGY1 Pharmacy Residency

Strong clinical decision-making and analytical skills

Excellence in communication and interpersonal abilities

Demonstrated leadership and team collaboration capabilitiesPreferred Qualifications:

PGY2 Solid Organ Transplant Specialty Residency

Clinical research experience
