TITLE: Certified Occupational Therapist Assistant (COTA)

QUALIFICATIONS:

Completion of an occupational therapy assistant program accredited by the American Occupational Therapy Association.

Current certification from National Board

Licensed to practice occupational therapy inConnecticut

Must be supervised by a licensed registered Occupational Therapist who is responsible for assessments, goals/objectives and triennials.

REPORTS TO: Director of Pupil Personnel Services

JOB GOAL: Function as a Related Service Provider to provide occupational therapy services delegated and supervised by a licensed occupational therapist. Under the direction of the occupational therapistfollows a service plan developed by the occupational therapist to improve, develop, restore or maintain students' active participation in self-maintenance, work, leisure, and play in educational environments.

PERFORMANCE RESPONSIBILITIES:

Assist with data collection and evaluation.

Provide direct service according to a written treatment plan that the occupational therapist develops.

Recommend treatment modifications to the occupational therapist based on students' needs.

Adapt environments, tools, materials and activities according to students' needs.

Communicate and interact with other team members, school personnel, and families in collaboration with an occupational therapist.

Participate in meetings as member of the multidisciplinary team.

Travel to and among schools to provide services.

Maintain treatment areas, equipment and supply inventory as the service plan requires.

Maintain record of service provided including Medicaid reimbursement forms.

Participate in policy and procedure development, in collaboration with an occupational therapist; participate in in-services and training.

Other duties as may be assigned by Supervisor.

TERMS OF EMPLOYMENT:

Ten-month year.

28.5 hours a week, Part-Time

Salary and work year to be established by the Board.
