Palliative Care Physician (Medical Director) Florida

Seeking a physician to lead a team of providers serving patients on the Hospice/Palliative Care Service. Work in collaboration with other palliative care team members in an active practice to deliver palliative care and related health care services to patients and families in a variety of settings. Provide timely consultation services in palliative care. Perform medical examinations and evaluations, diagnoses, treatment, follow- up, consultation and education. Interview patients and /or families, and assists the team in obtaining records, health histories, completes physical, psychological and functional assessments Write physicians orders in accordance with legal, ethical and organizational standards. Write orders pertaining to informed consents, diagnostic procedures and treatments. Provide complete and appropriate documentation of patient care services to support accurate billing of these services delivered and initiates reimbursement for services in a timely manner as determined by policy. Achieve productivity targets to include 6 consults or 12.5 RVUs per 8 hour work day. Billable accuracy percentage of 90% Prescribe medications as determined necessary. Facilitate clarification of patient and family goals of care. Consult with attending physician and interdisciplinary team to establish a written plan of care at intervals specified in the plan. Ensure provision of informed consent for CPR and DNR Provide oversight and guidance for ARNP/Pas. Provide oversight and guidance for CPC employed social workers and chaplains and those provided by partners to be part of interdisciplinary team. Facilitate access to appropriate supportive care services Provide educational sessions to advance palliative medicine among internal staff, medical community and community. Participate in interdisciplinary team meetings to review active cases. Perform other duties as required. Comply with applicable laws and regulations. Adhere to the practice of confidentiality regarding patients, families, staff and the organization Participate in on-call coverage. Provide coverage for physicians at institutions (privileges obtained) by being physically present at site and completing all consults ordered in a timely manner. Submit timesheets by 5th of each month for prior month. Rotate to one other service delivery location 2 times/year to broaden training. Lead select duties as part of Shared Governance Board Hospital requested education/meetings should not exceed 2 hours per week. Participation in hospital committees should at least be one but no more than two. Participate and perform to aid hospital partners to meet JCAHO certification.
