Medical Case Manager (Early Intervention Clinic) Lincoln Community Health Center, Inc., a federally qualified community health center located in Durham, NC is recruiting for a Medical Case Manager for the Early Intervention Clinic.

Position Summary: Responsible for the overall coordination and delivery of client-centered services that links clients with health care, psychosocial, and other services needed by patients, including treatment adherence services.

Reaches out and coordinates care and follow up of medical treatments for HIV/AIDS patients.

The medical case manager ensures timely and coordinated access to medically appropriate levels of health and support services and continuity of care, through ongoing assessment/reassessment of the client's and other key family members needs and personal support systems.

Actively supports the Lincoln Community Health Center's mission of service and education.

Knowledge, Skills, and Abilities: A general knowledge of Durham community and its resources is required.

Knowledge of HIV/AIDS through seminar and coursework required.

Interviewing, facilitation, and computer skills required.

Ability to communicate effectively, verbally, and in writing to diverse populations including patients, staff, and general public required.

Ability to perform with minimal supervision desired.

Valid North Carolina driver's license with a good driving record and reliable vehicle is required.

Education/Experience Required: Bachelors or advanced degrees in the areas of social work or health and human services.

Minimum of two years of previous work experience in the health field related to HIV/AIDS care.

Two (2) years of working in a community/public health setting.

General knowledge of the Durham community desired.
