The Primary Care Physician, Leadership Fellow will be part of an accelerated development for new clinical leaders within the organization dedicated to patient-centered, value-based healthcare, supporting our urgent pursuit of more good days for Seniors.

The initial focus will be on mastering the Core Model as a Primary Care Physician Leader-in-training primarily focused on the clinical & operational requirements during the first 6-12 months followed by several intensive leadership development programs & experiences to accelerate readiness and performance as an independent center clinical leader within 18 months.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES: * Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.

* Takes full accountability for patient care and outcomes and appropriately seeks consultation from internal specialists when needed (external specialists when internal is not an option), though will still stay involved in, and remain responsible for, the detailed care of the patient regardless of involvement of an internal or external specialist.

* Once mastery of the Core Model is established, plays an active role in the management of their center, the center's physicians, and will help cover for other providers who may be out for various reasons.

It is also expected that each PCP, Leadership Fellow, will take an active role as needed in recruiting patients for the center and additional providers for the company.

* Engages in Facilitated Practice and Leadership rounding both as a recipient and to support growth and development of other providers in center.

* Engages with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not). * Responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.

* Leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.

* Engages with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market for patients that are unable to come to the office-in hospital, SNF, LTC or homebound.

* Takes full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient.

* Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP while employed with the company.

Preliminary focus before moving on as Leader Fellow is to demonstrate mastery of the Core Model after which the incumbent will begin focus on accelerated leadership development and taking a greater leadership role in the center including, but limited to: * Pursues partnership attainment within the first two years to demonstrate mastery of the Core Model and begin to coach and develop other Primary Care Physicians to attain partnership.

* Assists the Associate Center Clinical Director and/or Market Clinical Director in the selection, training, onboarding, and coaching of new Providers within the center.

* Shadows current Associate Center Clinical Director and/or Market Clinical Direct to gain additional insights into leading centers.

* Actively engages and invests in leadership development by attending and participating in various leadership programs designed to prepare the PCP Leadership Fellow for their center assignment as the Associate Center Clinical Director at your center where you trained or within the Market as available or in another Market if open to relocation.

* Leads Super Huddle (SH) and Transforming Care Meeting (TCM) weekly, as well as supports Center Operations Director if the Center Clinical Director and/or Market Clinical Director are not available in the execution of these critical meetings, based on guidance from Market Clinical Director and Market General Manager.

Fills in as needed for center clinical leadership needs, including monitoring daily center census as part of joint center accountability for outcomes after the start of the Leadership development programs.

* Assists Center and Market leadership in the identification and action planning for key Clinical outcomes within the center and support coaching and training of other providers.

* Performs other duties as assigned and modified at manager's discretion.

KNOWLEDGE, SKILLS AND ABILITIES: * Advanced -level business acuity * Expert knowledge and understanding of general/core job-related functions, practices, processes, procedures, techniques and methods * Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other applications used in the company * Ability and willingness to travel locally as needed in their market, if applicable, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes * Must have openness to commuting to neighboring centers in the market when Associate Center Clinical Director roles become available or openness to relocation to other markets if desired * Demonstrates emotional intelligence and ability to recognize and respond appropriately to others in all interactions * Ability to communicate effectively by listening actively, adapting communication style to different audiences and situations.

* Ability to build trust and credibility through consistent, reliable actions that demonstrate integrity and respect for others * Ability to provide feedback and coaching that is specific and supports growth and improvement * Ability to manage time effectively by practicing prioritization, meeting deadlines, managing multiple priorities and delegating appropriately * Fluency in English, verbal and written.

There may be jobs in some centers that require fluency in other languages, and this will be made known at the time of application.

* Ability and willingness to travel locally, regionally and/or nationally up to 10% of the time; flexible to work evening, weekends and/or holidays as needed * Spoken and written fluency in English * This job requires use and exercise of independent judgment Other Responsibilities may include: * Collaborates with other multidisciplinary team members to analyze and evaluate current systems of health care delivery to identify and implement new practice patterns as appropriate.

* Participates in outside activities that enhance personal and professional growth and development.

* Initiates arrangements and writes orders for SNF discharges and completes appropriate paperwork.

* Works collaboratively with physicians, nurses, PT, social workers, family and key caregivers to transition the patient to a lower level of care as soon as medically appropriate.

* Introduces self to patient/family and explain nurse practitioner role.

* Facilitates patient/family conferences to review treatment goals, optimize resource utilization, provide family education and identify post-SNF needs.

* Enhances a collaborative relationship to maximize the patient's/family's ability to make informed decisions re: goals of care, palliative care and hospice.

* Facilitates discharge to appropriate level of care and uses preferred providers when additional services are required.

* Prescribes medication to patients based on State of practice.

* Other duties as assigned and modified at manager's discretion.

KNOWLEDGE, SKILLS AND ABILITIES: * Demonstrated record of consistently achieving clinical performance metrics in current role * Strong Critical Thinking and problem-solving skills * Excellent communication and interpersonal skills * Time management skills with the ability to work well under pressure * Must be caring and empathetic and have great listening skills * Must be detail-oriented, and able to pay close attention to patient charts, medications, and follow-up on details of patient care * Basic computer skills and some knowledge of Microsoft Office Suite * This position may require 50-75% of local travel EDUCATION AND EXPERIENCE CRITERIA: * MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required * Current active MD licensure in State of employment is required * A minimum of 1-year clinical experience in geriatric, adult or family practice setting preferred * Prior leadership experience from residency programs such as Chief Resident, geriatric fellowship or equivalent experience for externals * Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred, Board Eligibility is required * Once Board certified the incumbent will maintain board certification in their terminal specialty by doing necessary MOC, CME and/or retaking board exams as required * Must have a current DEA number for schedule II-V controlled substances * Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment We're ChenMed and we're transforming healthcare for seniors and changing America's healthcare for the better.
