Position Purpose:
The Licensed Vocational Nurse (LVN) delivers compassionate, client-centered nursing care within the Enriched Residential Care (ERC) program-a 48-bed facility serving ambulatory residents with comprehensive support services. The LVN promotes resident health and wellness through direct care, care coordination, and administrative oversight, while ensuring compliance with Title 22 and all applicable County, State, and Federal regulations.
This role requires a flexible, team-oriented approach and involves both clinical and administrative duties. The LVN works closely with the Program Administrator, Med Tech, Case Manager, CNAs, and senior leadership to ensure high-quality, coordinated care throughout the ERC facility.
Program Population:
The Crocker Residential Care is a licensed Residential Care Facility for the Elderly (RCFE) that provides 24/7 non-medical care and supervision to adults aged 60 and older living with chronic health conditions. Our program offers a home-like environment with support for activities of daily living (ADLs), medication reminders, and a focus on social engagement and independence. Some residents may experience mental health or substance use challenges, which can occasionally lead to behaviors such as raised voices, strong language, or angry outbursts. The core responsibility of every staff member at Crocker Residential is to support residents in overcoming obstacles and strengthening their coping skills. The Crocker Residential Care Facility is a pet- friendly environment. On occasion, employees may be working around friendly pets. Applicants should have no severe allergies to or significant fear of dogs.
Principal Responsibilities:
Address any staff or resident issues promptly and escalate to the Program Administrator as needed.
Provide facility oversight during assigned shifts.
Ensure appropriate and consistent documentation is completed as required during assigned shifts such as incident reports and shift logs.
Adhere to and enforce all safety and security protocols as needed, including knowledge of emergency preparedness plans.
Conduct client assessments to identify health needs, observe changes in condition, and support care planning and follow-up.
Conduct shift briefings and ensure proper handover between shifts.
Administer prescribed medications and treatments in accordance with provider orders and clinical protocols.
Support Certified Nursing Assistants (CNAs) to ensure quality and compassionate care practices.
Collaborate closely with the Med Tech to ensure accurate medication management and resident compliance.
Works in coordination with LVN supervisor, Program Administrator and Assistant Program Administrator and ensure that there are no operational issues during their assigned shifts.
Work in coordination with the Case Manager to support residents' psychosocial and medical needs, including follow-up on referrals, appointments, and service coordination.
Provide administrative oversight in the absence of the Program Administrator and Assistant Program Administrator including leading staff huddles, assigning duties, and responding to emergent operational issues.
Support facility compliance with Title 22 regulations, safety standards, and clinical documentation requirements.
Assist in the coordination of care transitions, including discharges and movement to higher levels of care when medically necessary.
Monitor medical supplies and support inventory management to maintain a clean and efficient clinical workspace.
Participate in interdisciplinary meetings, trainings, and quality improvement efforts to enhance program delivery and outcomes.
Maintain accurate and timely documentation in the Electronic Health Record (EHR).
Perform other duties as assigned.
Requirements:
Current and unrestricted California Licensed Vocational Nurse (LVN) license.
CPR/BLS certification required from America Heart Associate or American Red Cross.
Minimum of 3 years of LVN experience, preferably in community health, substance use treatment, recuperative care, or harm reduction settings.
Experience working with people experiencing homelessness, behavioral health challenges, or substance use is highly preferred.
Demonstrated commitment to trauma-informed, harm reduction, and client-centered principles.
Ability to manage complex, sensitive situations with empathy, discretion, and professionalism.
Strong interpersonal and communication skills, with a team-oriented mindset.
Proficiency in basic computer applications and electronic health records (EHR) systems.
Bilingual (English/Spanish) preferred but not required.
Ability to work nights and/or weekends may be required.
