Position Summary
The Mobile Health Nurse is a registered or licensed nurse who is key to providing support for our mobile health providers and safe care to our patients. The Mobile Health Nurse is key to strengthening operational quality and improving clinical patient outcomes. The Mobile Health Nurse assists in the care coordination of our patients' healthcare needs, and serves as a teacher, advocate, and navigator for these patients within the context of the mobile health team. This nurse will be responsible for providing direct nursing care, performing nursing assessments, participating in the continuity of patient care, being a servant-leader, and helping improve patient outcomes. The Mobile Health Nurse reports to the Director of Nursing and the Chief Clinical Officer of Operations and Providers.
Principal Duties and Responsibilities
1. Provides clinical and administrative support for providers within the care team model via nurse visits. This includes:
• Serving as a liaison between providers and their patients.
• Communicating abnormal lab results and treatment plan to patients as outlined by the provider and/or clinic protocols.
• Completing nursing tasks such as: checking INR level checks with Coumadin titration, BP/BG checks and medication titration as ordered by provider, transfer of scripts, patient education on chronic disease management, medication administration and pill organizer assistance, triage, pregnancy tests/counseling, HIV rapid tests and conveying test results.
• Executing nursing care under direction of the provider such as: IV infusions, medication injections, wound care and dressing changes, hygiene assistance and podiatry care
• Working with other members of the care team to coordinate patient care, referrals, DME orders, care management and hospital/ER follow-up
• Meet with patients needing medication refills under the guidance of the clinic nursing protocol
2. Directs, in conjunction with provider, the assessment of acutely ill patients who present to the mobile health team.
Using assessment and triage, the nurse will support patients in accessing the level of care they require.
3. Complete medication reconciliation and new patient intakes
• Nurse will meet with patients to discuss pertinent medical history, current active medications and chief medical concerns, and basic physical assessment. RN will review SBIRT, GAD/PHQ9, and mental health status exams when appropriate and place / read PPDs in shelters that require them for entry.
4. Motivational interviewing / education for MAT/psych med management and assisting with assessment of s/s following starting a new medication
5. Medical Respite Rounding
• Schedule and evaluate each respite patient on the rounding list. Write a care note in EPIC. If a patient is not present on-site, then call the patient and leave a note in EPIC. Responsibilities include but are not limited to:
• Wound care
• O2 weaning
• Medication administration - when direct oral therapy needed, for acute medication needs, and for IM medicine administration.
• Medication reconciliation, refilling meds
• Education about diet, correct medicine administration, physical activity, diabetes management, etc.
• Triage: assessing clinical status, if decompensation has occurred, and correct level of care needed. Communicating change in care plan, if necessary, to on-call respite provider.
• Update the Respite Rounding List, when needed
6. Participates in care team meetings, monthly nurse meetings, clinic-wide quarterly meetings and other meetings as
directed by the Director of Nursing.
7. Documents all activity appropriately in the Electronic Medical Record.
8. Participates and supports new efforts within the Mobile Health department including clinical care as well as policy and procedure development.
Required Skills or Abilities*
• Strong patient assessment skills required including ability to triage patients. Ability to use the nursing process to guide patient encounters and care.
• Communicate effectively with patients in a manner that protects their confidentiality and is sensitive to their culture and physical/emotional/spiritual condition.
• Flexibility to adapt to changing or stressful conditions, including unanticipated changes to working schedules or locations.
• Good physical stamina and an ability to be standing/active/in motion most of the day. Strength and dexterity to move patients, handle equipment, and move quickly within the clinical area to fulfill job responsibilities.
• Good interpersonal skills are a necessity, including an ability to work well with the variety of ages, cultures, and temperaments represented among LCHC staff and patients, treating others with kindness and professionalism in all they do.
• Commitment to demonstrating personal integrity through punctuality, honesty, an ability to follow instructions, proper attention to detail in all work matters, and a willingness learn from others.
• An ability to work independently, take initiative, and set priorities in accordance with the needs and mission of the mobile health team.
• Clear and concise written and verbal communication skills for communicating coherently and professionally with patients and co-workers.
Required Knowledge, Education, Experience or Licensure/Registration
RN licensure in the state of Illinois
CPR certification
Clinical experience preferred (desirably in a clinic, acute care, or triage setting), but new graduates with an interest in community health are encouraged to apply
Knowledge of Spanish is helpful
