PURPOSE OF JOB:
Under the general direction of the hospital pharmacy, obtains medication histories for admitted patients arriving through the Emergency Department. Contributes to the Hospital's SERVICE values by interviewing patients and patient support network, researching EMR systems and contacting other resources to collect the most comprehensive and accurate patient home medication list as part of the admissions process. Interacts with and provides medication history and other related patient information to nurses, physicians, pharmacists and other medical professionals as appropriate. This staff position will have an impact on improving safety and quality of our patients by providing physicians and physician extenders with a comprehensive home medication list.
ESSENTIAL FUNCTIONS:
1. Utilizes knowledge of medications and sources of medication data to accurately compile a comprehensive best possible medication history (BPMH) for patients admitted to Doylestown Hospital.
2. Accurately enters a patient's medication history and pharmacy data into Meditech.
3. Coordinates/assists with deliveries of code trays to the Emergency Department.
4. Interacts with and provides medication history and other related patient information to nurses, physicians, pharmacists and other medical professionals as appropriate.
5. Actively participates in ongoing quality improvement programs focused on medication reconciliation.
JOB CRITERIA AND PERFORMANCE STANDARDS:
Utilizes knowledge of medications and sources of medication data to accurately compile a comprehensive best possible medication history for patients admitted to Doylestown Hospital.
Works under the supervision of a pharmacist.
Interviews a patient and/or patient support network prior to admission to Doylestown Hospital and obtains the best possible medication history (BPMH).
Provides high levels of customer service for patients and caregivers while obtaining an accurate medication list.
Investigates other sources of medication data (patient's retail pharmacy, mail order pharmacy, physician(s), etc.) if needed to obtain BPMH.
Accurately enters a patient's medication history and pharmacy data into Meditech.
Ability to navigate through Meditech and EMR.
Enters medications into a patient's profile using appropriate procedures.
Enters a patient's pharmacy/pharmacies into Meditech.
Coordinates/assists with deliveries of code trays to Emergency Department.
Follows departmental procedures for deliveries and coodinates with main pharmacy and Emergency department to have code trays delivered in a timely manner.
Retrieves unused or discontinued medications from patient care areas and returns them to the pharmacy.
Acts as Pharmacy liaison to the Emergency Department.
Interacts with and provides medication history and other related patient information to nurses, physicians, pharmacists and other medical professionals as appropriate.
Works closely with ED Providers and nursing to assure all patients being admitted to the hospital have a BPMH completed prior to admission.
Coordinates with admitting physicians/physician extenders as needed to clarify any medication discrepancies uncovered in the admission process.
Actively participates in ongoing quality improvement programs focused on medication reconciliation.
Participates in multidisciplinary workgroups to improve medication reconciliation process.
Identifies and reports problems with associated with medication reconciliation.
Effectively communicates with multiple levels of the organization.
Job Qualifications:
V. QUALIFICATIONS:
Education: High School Graduate or Equivalent, Pharmacy Technician Certification desirable
If Certified, must have current Pharmacy technician certification
Experience: 2 years of previous pharmacy technician experience in clinical setting or 5 years previous experience in retail pharmacy.
