Population Health Representative Lincoln Community Health Center, Inc., a federally qualified community health center located in Durham, NC is recruiting for a Population Health Representative.

OCCUPATIONAL SUMMARY The Population Health Representative serves in an administrative role within the Department of Population Health, collaborating with all departments to strengthen the patient-clinician relationship by supporting the planning and coordination of care across the healthcare spectrum.

This role is primarily telephone-based, involving making and receiving phone calls, conducting targeted outreach to engage patients, managing the hospital follow-up line, and promoting access to care.

The comprehensive care provided by the Population Health Representative aims to promote better health, longer lives, higher patient satisfaction, and less expensive care.

MINIMUM QUALIFICATIONS * High School diploma or equivalent.

* A minimum of one-year experience in a healthcare setting.

* Knowledge of medical terminology.

* Experience working for a non-profit community health clinic and/or social service organization.

EXPERIENCE * At least one year of Ambulatory Care experience is preferred.

* Must have a working knowledge of HIPAA and Joint Commission Patient Care Standards.
