Overview Position Title: COMMUNITY HEALTH WORKER SUPERVISOR Job Summary: The Community Health Worker (CHW) Supervisor will be a member of the TN CEAL research team and will supervise frontline CHW personnel to screen, enroll, support, and track progress of participants in the study who have multiple chronic diseases and live in areas designated as medically underserved or socially vulnerable.

The CHW supervisor will supervise the activities of community health workers (CHWs) and provide them with adequate coaching and support to ensure the quality of their work and the accuracy of their documentation and reports.

Primary Responsibilities: The following are primary roles and responsibilities of the Community Health Worker Supervisor: • Collaborates with TN CEAL leadership to develop and implement the TN CEAL Patient Activation Intervention.

• Provides day to day supervision of community-based activities carried out by CHWs for TN CEAL.

• Ensures high quality and timely implementation of community-based activities provided by the CHWs for TN CEAL.

• Builds and maintains strong cooperation with TN CEAL partners to keep them informed and actively involved in community health activities.

• Builds and ensures strong partnership with health facilities that serve TN CEAL participants.

• Ensures safe and accurate use/storage of TN CEAL project equipment, marketing materials, documentations tools, and other resources provided.

• Collaborates with TN CEAL leadership to develop and implement ongoing training and continuous learning opportunities for CHWs.

• Establishes a monthly work plan and calendar of supervision activities and shares it with CHWS, other TN CEAL personnel, and key partners.

• Ensures supervision of each CHW under his/her responsibility every month including: o Conducting at least one supervision visit every month to observe CHW interaction such as a visit to a community group, a home visit, or some other patient activation activity.

o Coaching and supporting CHWs to plan their weekly and monthly activities.

o Evaluates CHWs' performance, provides feedback, and co-develops recommendations to solve problems and improve CHW performance • Collects monthly activity reports from each CHW and under his/her responsibility every month.

• Ensures quality review of the reports provided by CHWs.

• Submits monthly reports to TN CEAL leadership, consolidating information from the monthly activity report compiled by all CHWs under his/her supervision.

• Using a handheld device or other technology, submit clear and accurate data collection forms for all work performed.

• Comply with all study protocols, including those regarding confidentiality of client information.

• Complete employee orientation and all mandatory program trainings to guarantee appropriate competency in program requirements.

• Participate in all staff and program meetings, site visits with funders, required network events, and mandatory training.

• Keep the designated supervisor(s) informed of all problems, challenges and conflicts related to all assignments associated with the job.

• Perform other duties assigned by designated supervisor(s). Required Qualifications: • Experience: 3-5 years of supervisory experience of CHWs or similar role in a health or social agency setting.

Experience working with people of different races, ethnicities, cultures, religions and socio-economic backgrounds.

Prior experience as a CHW and in delivering grant funded programs is preferred.

• Education: A bachelor's degree in community health management or related field.

A master's degree is preferred.
