MCM Support Specialist supports clients in accessing and maintaining care by assisting with applications, processing intakes, and coordinating medical record requests. Responsibilities include tracking and scanning documentation, monitoring referrals and waitlists, and supporting educational events. The position also assists with data entry, reporting, and other program support tasks to ensure efficient and timely client services.Key Responsibilities:
Supports MCM clients with HDAP application in order to maintain their medications
Follows up with HDAP Program re: status of applications and submitting any necessary documents
Supports medical case managers with obtaining ct medical records every six months through HIV medical providers.
Creates and maintains a tracking chart of medical records requests in order to compile data related to turnover time for records requests.
Responsible for scanning received medical records into cts EHR document library.
Responsible for supporting APD with monitoring referrals and following up with waitlist to identify if cts are still interested in services.Qualifications:
Required:
A High School diploma or GED.
Valid Driver’s License and reliable transportation as this position may require outreach
1 year of administrative experience preferred
Excellent verbal and written communication skills; interpersonal skills; crisis intervention skills; organizational skills; attention to detail; computer skills; good customer relations skills; knowledge of legal and ethical issues pertaining to the profession. Experience using excel, google sheets and must have data entry experience. Experience with EHR is a plus.
Preferred:
Bilingual (English/Spanish) preferred but not required.

Summary of Benefits:
Salary Range: $18.25 – $24.25
Based on years of experience, level of education, specialty and licensure/certifications. Additional Compensation for advanced education for qualifying positions.
