Purpose of Position

JOB SUMMARY

The job duties of the Medical Services Screener (MSS) in the Planning and Strategy Division of Public Health include but are not limited to performing delegated professional assessment tasks in locations including an office, health department clinic, school, or home environment. The MSS will complete Children's Long-Term Services Functional Screens, referrals to the Birth-To-Three program, may provide intake and referral services, care coordination services, community and family education and other tasks to promote the provision of needed services to Children and Youth with Special Health Care Needs (CYSHCN) and their families, especially in the areas of mental health and other special health care needs. Services are to be provided in a responsive, courteous and professional manner to County residents and the general public.

Essential Duties and Responsibilities

DUTIES AND RESPONSIBILITIES

The duties described below are indicative of what the MSS might be asked to perform. This job description is to incorporate any county ordinances created for the position of MSS. This is not an exhaustive list of job responsibilities, and therefore, other duties may be assigned:

Administrative Functions:

Schedule and conduct Children's Long-Term Care functional screens and follow-up to referrals to the Children's Resource Center - West (CRC) . Follow appropriate procedures for obtaining necessary medical and school records for screening and documenting activities and outcomes. Initiate functional screen referrals from parents, Chippewa County Department of Human Services, physicians, schools, hospitals, the Aging and Disability Resource Center (ADRC), and other community agencies.

Process Birth-To-Three referrals.

Complete required documentation for record-keeping and billing of services.

Work in partnership with DHS and CRC staff to provide direct client services, education, technical assistance, identifying additional CYSHCN resources, and health benefits counseling as needed.

Enter all required client and system data into the designated data collection system as required by the Wisconsin Division of Public Health. Enter all required client data into the Nightingale Notes Public Health electronic record, the CWDA Intakes spreadsheet, and the CLTS-CCS Status form as per procedure.

Complete and submit timely reports and documentation according to agency policy.

Participate in continuing education programs.

Display a professional attitude when interacting with other community agencies, clients, family members, and health care professionals.

Work with agency personnel and others to promote and provide agency services.

Evaluate own needs for professional development and plan to meet those needs.

Participate in department-wide performance management/quality improvement and strategic planning activities as assigned.

Participate in department-wide preparedness activities, including National Incident Management System (NIMS)/Incident Command System (ICS) training and exercises.

Assist in Public Health Emergency Response as assigned.

Support and participate in public health accreditation, re-accreditation, 140 reviews, annual reports, and department strategic plans as assigned by the manager.

Assist the Community Health Division with vaccinations as needed.

Community Relations Functions:

Attend CLTS weekly division meetings, in-service meetings and educational conferences as established by the agency.

Customers:

CRC program consumers (parents, foster parents, other family members, children and youth with special health care needs)

General public/citizens requesting information pertaining to children with special health care needs

CRC state, regional and local community partners including those from education and health care sectors

University partners and interns

Board of Health and Human Services

Team Members:

Employees of the Planning and Strategy Division

Employees of the Department of Public Health

Employees of the Department of Human Services (Children's Long-Term Support (CLTS), Birth to 3, Economic Support, ADRC)

Minimum Qualifications

QUALIFICATION REQUIREMENTS

To perform this job successfully, an individual must be able to perform each duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required at the time of hire or for the continuation of employment.

EDUCATION AND/OR EXPERIENCE:

Bachelor's degree in social work, registered nursing, or a related health care field is preferred.

Associate's degree in a medical field or Licensed Practical Nursing with a minimum of two (2) years professional work experience required.

Two (2) years or more of professional work experience in the mental health field is preferred.

Must meet statutory requirements to perform functional screens in the state of Wisconsin, including successful completion of Children's Long-Term Functional Screen training within two (2) weeks of the date of hire.

Must maintain professional credentials to practice in Wisconsin, if applicable.

Experience working with families with children who have special needs is preferred.

A valid driver's license is required.

A cell phone is recommended.

Chippewa County offers a comprehensive benefit package that includes the following:

Health Insurance - The County's Health Plan's Third Party Administrator (TPA) is AmeriBen. The network is Anthem BluePreferred network including most medical providers in the Chippewa Valley. The plan has a $3,000 - Individual and $6,000 - Family deductible with an HRA funding. Individual and Family coverage has a $0 monthly premium for General Municipal Employees (GME). To determine monthly premiums for WPPA Employees, please refer to the WPPA Labor Agreement 2023-2025.

Health Reimbursement Account (HRA) - The County contributes money on the employee's behalf and can be used for qualified Medical, Dental, or Vision expenses. All employees and dependents enrolled on the Health Insurance Plan qualify. A total of $500 per person enrolled on the plan, up to a max of 4 - $2,000 annually.

Near-Site Clinic - All employees and dependents enrolled on the Health Insurance Plan have access to services at ReforMedicine. Services with ReforMedicine are FREE!

Care Navigation - All employees and dependents enrolled on the Health Insurance Plan have the ability to utilize Alithias. Alithias will help save time and money by answering questions about Healthcare benefits, medical bills, help find high quality, in-network Providers, and high value Providers for all non-urgent clinic services.

Musculoskeletal Partner - Physical Therapy - All employees and dependents enrolled on the Health Insurance Plan have access to services at Doctors of Physical Therapy. Services with Doctors of Physical Therapy are FREE!

Wisconsin Retirement System (WRS - ETF) - Employees (GME) will begin contributing 6.95% (2025) immediately to the state pension program through WRS. The County will match the employee contribution, 6.95% for a total of 13.9%. This percentage may change each year based on state requirements. To determine WRS percentages for WPPA Employees, please refer to the WPPA Labor Agreement 2023-2025 or contact Chippewa County Human Resources.

Additional Insurance Plans Offered: Dental, Vision, Life, Accident, and Critical Illness, Short-Term (GME only) and Long-Term Disability Insurance; Flexible Spending and Dependent Care Accounts.

PTO - accrual equal to 7.09 hours of PTO per pay period (26 pay periods in a year). An employees total accrued PTO may not exceed four hundred eighty (480) hours. PTO hours roll over year to year.

Holidays - 10 paid holidays per year.
