GENERAL DESCRIPTION:

Primarily Remote-16 hours per week

This role focuses on improving patient outcomes through virtual coaching, personalized health education, and the creation of culturally and linguistically appropriate educational materials. This position will play a vital role in empowering patients to take an active role in their health and well-being, particularly in underserved communities.

DUTIES AND RESPONSIBILITIES:

Telehealth Coaching

- Conduct one-on-one or group telehealth coaching sessions to provide guidance on nutrition, fitness, chronic disease management, mental health, and lifestyle changes.

- Develop and implement individualized health and wellness plans tailored to patients’ needs and goals.

- Serve as a supportive and motivational resource to help patients navigate their healthcare journey.

- Utilize evidence-based practices to educate patients on managing conditions such as diabetes, hypertension, and obesity.

- Partner with primary care providers and care teams to ensure care plans align with clinical objectives.

Patient Education Creation

- Create engaging, accessible, and culturally relevant educational materials for a variety of health topics, including chronic disease prevention, mental health, nutrition, and preventive care.

- Design resources in multiple formats, such as brochures, videos, infographics, for social media posts, tailored to patients’ literacy levels and technological access.

- Collaborate with clinical staff to ensure accuracy and compliance with medical guidelines and health literacy standards.

- Stay informed about emerging health trends and develop proactive educational campaigns to address community needs.

Program Development and Evaluation

- Assist in the development and implementation of health promotion programs targeting specific patient populations.

- Collect, track, and analyze patient engagement data to assess the impact of educational initiatives and coaching sessions.

- Identify barriers to care and recommend strategies to improve patient access and outcomes.

- Educate Chronic Care Management staff so they have aligning knowledge and can serve as a health coach.

- Directly supervised by the Chronic Care Manager Coordinator

Interrelationships:

- The employee will interact with patients, providers, support staff, administration, Chronic Care Management staff, staff in quality, and EHR Clinical Specialist.

REQUIREMENTS:

In addition, this position requires:

Education:

- Bachelor’s degree in health education, public health, nutrition, nursing, or a related field

Licensure and Credentials:

- Certification in health coaching (e.g., CHES, NBHWC, or equivalent) preferred.

Work experience:

- Experience working in telehealth or digital health environments, particularly with underserved populations, is highly desirable.

Skills:

- Strong communication and motivational interviewing skills.

- Ability to create engaging, patient-centered educational materials.

- Proficiency with telehealth platforms and digital tools.

- Understanding of health disparities and cultural competence in care delivery.

- Analytical skills to measure the effectiveness of patient education and coaching programs.

Physical Abilities:

- Have the hand-eye coordination and manual dexterity needed to operate a computer, telephone, copier, standard office equipment, and medical equipment.

- Required to talk and have a normal range of hearing and eyesight to be able to collect data and record where appropriate (i.e. computer and/or paper).

- Vision abilities required for this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus

Work Environment:

- Reliable internet access and private, HIPPA-compliant remote work environment.

- Remote, work-from-home position with structured daily schedule.

- Occasional travel to clinics, training, or community events may be required.

- Computer, phone, and secure access to EHR will be provided.

- Requirements for out-of-town and/or overnight travel are minimal.
