Position Summary:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our home health team. This is a rewarding opportunity to deliver high-quality, one-on-one care in a home setting while enjoying flexible scheduling and a strong work-life balance.
Key Responsibilities:
As an LPN with Community Care, Inc., you will:
Provide one on one patient care in the patient home.
Monitor patient progress and collaborate with the healthcare team to update care plans.
Educate patients and families on health management and wellness strategies.
Ensure accurate and timely documentation of patient interactions and care provided

Qualifications:
Active LPN license in the state of Pennsylvania.
Current CPR certification.
Reliable transportation and valid driver's license.
Minimum of six months of relevant patient care experience.
Ability to work independently and manage a flexible schedule
