This position will entail working with children, youth, and families with complex needs, who would benefit from intensive, highly individualized planning and care coordination with the goal of moving the families towards independence and success. You will identify needs, develop a support team, develop individualized and strengths-based plans, and coordinate services and supports. The Wraparound Facilitator will work closely with all disciples on the team, to include the educational system if needed, with the purpose of optimizing collaboration and integration of care. The team will promote the strengths, voice, and culture of the family to build confidence and experience success at home, in school, and out in the community. This role is primarily remote, with flexibility in creating your weekly schedule.

Wraparound Facilitator Responsibilities:

Facilitate the development of an individualized wraparound plan of care for each child/family that includes clear needs statements, strength-based strategies, and specific outcomes

Build strong rapport with the client and any involved caregivers and family members by communicating effectively, maintaining a positive attitude, being culturally conscious, and exhibiting empathy and a desire to help

Work with youth and families living with various mental and intellectual disabilities and families in at-risk communities with various levels of system and legal involvement

Coordinate and facilitate ongoing child and family wraparound team meetings

Keep the family and team members informed on progress toward achieving the specified goals in the plan of care

Continuously reassess the needs of the child and family with the team and modify the plan of care as needed

Act as liaison when new services or resources are needed

Support in the referral of clients to resources like psychiatrists, treatment facilities, or community services as

Maintain an empathetic and professional relationship with children and families that strives to keep families engaged in all services

Maintain good working relationships with support networks, government resources, and community resources

Provide written detailed progress notes for the case manager and the family every six months

Caseload will be a maximum of 10 children/families

Wraparound Facilitator Skills and Qualifications:

Excellent communication skills (active listening, motivational interviewing, nonviolent communication, mediation)

Experience working with a diverse range of professionals (educational, mental, medical, legal, human services)

Excellent time management skills; organized and able to prioritize

Comfortable in a fast-paced environment with multiple clients

Able to organize and manage large amounts of files, schedules, dates, and information

Calm and cool-headed with a positive attitude

Self directed, motivated, and able to work well independently

Empathetic and supportive with mentorship and leadership skills

Proficient computer skills, including Microsoft Office and Google Workspace

Dedicated to the success of children and family teams

Knowledge of systems interacting with youth and their families (human services, legal, probation, etc.)

Requirements:

Master's degree in Counseling or related subject

Bachelor's degree & relevant wraparound experience may be considered

Complete the National Wraparound Implementation Center Series of Wraparound Trainings within the first year of employment

Obtain and maintain CANS (Child and Adolescent Needs and Strengths) certification, including completing all required training and assessments

Pay Range

$25-$60/hour, based on licensure and years of experience

Initial 90 day salary may be offered while caseload is being established
