Job Title: Settlement House Case Manager

Position Status: Full-Time (hours vary based on youth school schedules)

Position Classification: Non- Exempt

Remote: No

Department: CAPP

Immediate Supervisor: CAPP Director

Salary: $40,000/annually

Job Summary:

The Settlement House Case manager position is a multifaceted position that incorporates Case Management for at risk youth and their families while additionally facilitating youth program dedicated to the health growth of the youth who participate. The Settlement House Case Manager hosts weekly groups that focus on STEM, social well-being, and emotional health. The Settlement House Case Manager is additionally trained in Aggression Replacement Therapy, and other evidence-based curricula as needed.

Supervisory Duties:

This position has no supervisory duties.

Essential functions: Percent of time:

Screening and Social Care Navigation 40%

Assessing youths' physical and mental wellness, needs, preferences, and abilities and using these to develop tailored care plans

Working with clients, family and friend support networks and health care professionals to put care plans in place

Listening to clients' concerns and providing therapeutic intervention as required

Evaluating clients' progress periodically and adjusting as needed to improve outcomes

Following up with discharged clients to ensure they were satisfied with services, and they are still in good physical and mental health

Builds respectful, person-centered relationships with youth and young adults using a trauma-informed approach. Welcomes and works with youth of all backgrounds including LGBTQ youth.

Demonstrates increasing knowledge and understanding of youth and young adults' developmental opportunities and challenges

Acts as an advocate by referring participants or parents to services available in the community as needed (e.g., family planning, health insurance, etc.)

Facilitation of Evidence Based Programming 40%

Demonstrates comfort, confidence, objectivity, and full support for educating youth and young adults about self-esteem, health, and well-being.

Demonstrates sensitivity to and cultural literacy about different opinions, sentiments, and beliefs.

Facilitates evidence-based curricula with fidelity to promote healthy wellbeing, and the ability to respond to everyday stressors without using aggression.

Administers pre- and post-surveys collecting student data from program sessions.

Implements, coordinates, and monitors activities across multiple systems to improve health and quality of life outcomes for youth and their families.

Works in a professional manner with the adult staff at sites where programming is to occur, building good working relationships with client system representatives.

Works with teams to provide department events for youth.

Data Management/Administrative 15%

Understands contractual requirements and current status of grants and submits informed and thorough monthly reports

Conducts process evaluation and maintains accurate records of group sessions at a variety of community locations. including Buffalo Public Schools, faith-based sites, and other locations.

Recording clients' progress including charting referrals, home visits and other notable interactions

Any other job responsibilities directly assigned to you by your supervisor. 5%

Education:

Bachelors from an accredited college or university and one year of full-time experience in a recognized youth service agency

Required Skills/Abilities:

Valid NYS Driver's license with evidence of good driving record

Demonstrated comfort and skill in public speaking to small and large groups, clearly and concisely explaining ideas, in a manner that keeps people's attention

Strong verbal communication skills and comfort with communicating in person and by phone, email, text message, and other mediums.

Ability to keep written case notes and submit required documentation.

Comfort with computers and web-based systems. Familiarity with databases, Microsoft Office, and Google Apps a plus.

Self-directed and able to work well independently as well as part of a team.

Creative thinking, sharing ideas that will help ensure project success.

Demonstration of a helpful, positive, and nonjudgmental approach, showing respect and fairness for each client and for all stakeholders.

Positive attitude with motivation to provide interactive and compassionate experience to those in our programs.

Must exhibit a passion for BFNC's vision, mission and values and act as a role model

Must be able to work sensitively and effectively with individuals of diverse educational, socioeconomic, and cultural backgrounds.

Ergonomics:

Sitting, standing, stooping, bending, standard lifting up to 35 pounds, carrying, repetitive movements. Ability to operate and repair machines; prolonged periods of standing, walking, bending, and stretching. Must be able to access and navigate the site and go up and down stairs. Must be able to work in varied weather conditions and outdoors during summer months. All the ergonomic requirements must be performed without significant risk of injury to oneself or others or otherwise demonstrated or explain how one can perform essential functions of the position listed above with or without accommodations.
