Job Summary:
Coordinates patient care services and resources to assure optimal clinical quality and financial outcomes
Works collaboratively with the Case Management Coordinator and other health care team members
Uses screening mechanisms to identify patients who are at risk for lengthy inpatient stays, complex clinical issues, high resource use or repeat admissions due to chronic diseases. Intervenes within 24 hours of patient admissions. Documents according to established guidelines. Hospital discharge planning experience is required.
Educational Requirements/Preferences:
Bachelors Degree in Behavioral Health, Social Work, or related field
Licensure/Certification/Registration: Maryland Social Work licensure required or LSWA. Current Maryland LCSW preferred
Experience/Requirements:
Experience as a social worker in a hospital environment with discharge planning experience.
