About the Division The Division for Community Living (DCL) develops and manages a comprehensive and coordinated system of community services for seniors and individuals with disabilities.
Their pre-admission review and long-term care consultation program facilitates consumer access to available options.
Staff develops and furnishes operational oversight of services and programs funded through home and community-based services (HCBS) as well as through state funds and grants.
Some of these programs include PASSPORT, the Assisted Living Waiver program, and Program for All-Inclusive Care for the Elderly (PACE). PASSPORT is a Home and Community Based (HCBS) 1915(c) Medicaid waiver program that provides Medicaid-eligible individuals over the age of 60, living in the community and meeting a nursing facility (NF) level of care, with the services and supports they need to age safely with dignity.

Participant directed care is an approach to delivering person-centered care in Ohio's PASSPORT Medicaid Waiver Program.
Participant directed care is a philosophy of service delivery that recognizes the individual is the one who knows their needs best and is designed to give individuals more choice and control over how, when, and by whom they receive their services.
The Participant Directed Care Manager is part of the Division for Community Living's PASSPORT Operations Team and is responsible for: * Overseeing the day-to-day operations of participant-directed services.
* Leading the implementation and monitoring of the participant directed strategic plan.
* Developing program resources and training materials.
* Monitoring the State's Financial Management Service (FMS) vendor contract and activities in collaboration with State partners.
* Supporting PASSPORT operations.

Completion of graduate core program in business, management or public administration, public health, health administration, social or behavioral science or public finance; 12 mos.
exp.
in the delivery of a health services program or health services project management (e.g., health care data analysis, health services contract management, health care market & financial expertise; health services program communication; health services budget development, HMO & hospital rate development, health services eligibility, health services data base analysis). * Or 12 months experience has Medicaid Health Systems Specialist, 65293, may be substituted for the experience required, but not for the mandated licensure, if required.

-If position oversees assessment or reassessment of clinical appropriateness of services &/or payment policies &/or related issues in regards to Medicaid health services delivery, incumbent must also have current & valid license as registered nurse as issued by Ohio Board of Nursing, pursuant to Sections 4723.03-4723.09 of Ohio Revised Code.
