The Outreach & Data Coordinator is responsible for providing services and support to assist senior clients; maintains databases and files; home visits to determine eligibility for services and extensive telephone contact with clients.

Duties:

Enter new client applications into database for CSFP and UFB box programs

Work alongside UFB box coordinator to ensure clients turning 60 are setup on CSFP

Maintain & update client records for UFB food box, CSFP box programs

Schedule client appointments for home visits and/or office meetings

Schedule and complete in-home visits for UFB box re-certifications

Perform monitoring reviews for CSFP distribution sites as required.

Assist training for UFB box and CSFP box programs as needed.

Maintain a clean company car, and filled up with gas. Turn in receipts to accounting

Meet with clients in office, at home, or over the phone

Conduct client "wellness checks" if we are unable to contact client by mail or by phone

Conduct on-site CSFP re-certification if we are unable to contact client by mail or by phone

Assist with following up on phone calls to potential clients that have called and requested applications as well as provide phone coverage when other CSFP or UFB Box staff are out of the office.

Run the "new client daily change" CSFP and UFB Box reports daily to review for data integrity

Determine client eligibility for services and which options will best meet needs

Provide coordination assistance for UFB clients in need of help beyond standard intake for food services and referral to other agencies for assistance

Continual efforts to reach out to new clients, utilizing creativity, follow up with community contacts, presenting CSFP and UFB food service information to the public, organizations, etc.

Other duties as assigned

Qualifications

Directly related work experience, preferably with seniors

Familiarity with community resources for meeting a wide variety of client needs

Valid Utah Driver's license and ability to drive in service area; must provide MVR and proof of insurance to drive personal vehicle for business if necessary

Must be able to communicate effectively and persuasively, in writing and verbally

Familiarity with word processing, spreadsheet and database software applications

Able to relate to seniors and people with disabilities in a professional and caring manner

Special Considerations: (Environment, Working Conditions, Travel, Physical Requirements, etc.)

Some of the work is in the homes of clients and potential clients of UFB Senior services and the CSFP program. Patience, understanding, and ability to put clients at ease is essential. The Outreach /Data Coordinator needs to have a broad knowledge of available services along with flexibility and creativity in problem solving and finding ways to provide assistance. Working directly with clients also requires excellent communication skills.

Physical Requirements:

Work is either in an office setting or in clients homes/senior centers. There is walking; standing; bending; carrying of light items such as papers, books, driving an automobile, etc. Outreach Coordinators may also deliver food boxes and should be able to lift up to 40 lbs. During peak seasons, all UFB staff may be asked to assist with receiving donations, picking up donations or other customer service work outside the normal scope of duties.
