Director of Social Work – Skilled Nursing Facility

We are seeking a compassionate and experienced Director of Social Work (DSW) to join our leadership team at our Skilled Nursing Facility. The DSW plays a vital role in ensuring our residents and their families receive the highest quality of emotional, social, and psychological support throughout their stay.

Responsibilities:

Lead and manage the Social Work Department, including supervising social work staff.

Support residents and families with adjustment to facility life, care planning, and discharge planning.

Conduct assessments, develop care plans, and maintain accurate documentation in compliance with federal, state, and facility regulations.

Act as a liaison between residents, families, and interdisciplinary care teams.

Provide education, counseling, and advocacy to ensure residents’ rights and psychosocial needs are met.

Participate in Quality Assurance and facility meetings as part of the leadership team.

Qualifications:

Master’s degree in Social Work (MSW) required.

Current state licensure as a Licensed Master Social Worker (LMSW) or equivalent.

Minimum 2 years of supervisory experience in long-term care or a healthcare setting preferred.

Strong leadership, organizational, and communication skills.
