Shifts: Day Shift 5x8-Hour (08:00 AM – 04:30 PM)
Job Summary: Providence Santa Rosa Memorial Hospital is seeking an experienced and highly skilled Registered Nurse – Case Manager for a 13-week travel assignment. The ideal candidate will have a strong background in acute care hospital case management, with the ability to perform care coordination, discharge planning, and needs assessments in a fast-paced environment.
Key Responsibilities:
Coordinate patient care and discharge planning in collaboration with interdisciplinary teams
Organize and participate in family meetings
Develop and execute comprehensive treatment plans
Evaluate patient needs and arrange DME, SNF, and ambulance transport independently
Utilize InterQual criteria to determine medical necessity
Ensure compliance with CMS, HIPAA, and other regulatory standards
Document all activities in Epic (required EMR system)
Manage case load with a 1:20 patient ratio
Float as required between units or facilities within the state
Required Qualifications:
Licensure: Current RN license in the state of California
Experience:
Minimum 2 years of recent acute hospital case management experience
Must have relevant experience within the last 2 years (HARDSTOP requirement)
Travel nursing experience required
Certifications:
BLS (Required)
Preferred: Certifications in utilization review, disability case management, etc.
Charting System: Proficient in Epic
Skills:
Discharge planning
Needs assessment
Prior authorizations
Admission criteria and care coordination
Familiarity with InterQual
Experience with HMOs, IPAs preferred
