As the 11115 Waiver Enhanced Navigator for Catholic Charities of Delaware, Otsego and Schoharie Counties, you will play a pivotal role in individuals' lives, making a significant impact through:
Conduct screenings with Medicaid eligible individuals for services under the 1115 Waiver
using the approved ACH screening. Ensure all demographic information is accurately collected and member consent is properly documented.
Ensure timely and accurate data entry of all services, interactions, case notes, and outcomes in the Unite Us platform in accordance with 1115 Waiver requirements
Provide education to the community, referral partners, and clients regarding eligibility criteria and available services under the waiver.
Participate in all required trainings associated with the 1115 Waiver, including but not limited to: Unite Us platform, trauma informed care, culturally and linguistically appropriate services, HIPAA, and compliance.
Collaborate with other Case Managers and Catholic Charities team members to ensure coordinated, client centered service delivery
11115 Waiver Enhanced Navigator Minimum Salary: $22.00 / Hour
11115 Waiver Enhanced Navigator Maximum Salary: $22.00 / Hour
11115 Waiver Enhanced Navigator Schedule: Monday – Friday 8:30AM – 4:00PM (35 hours per week)
Qualifications:
Associates Degree in Human Services or related field
1-3 years of experience in mental health, substance use, or human services
