Case Manager - Utilization Review

Schedule / Shift

3x12's: Saturday, Sunday, Monday 7am - 7pm

4x10's: Friday, Saturday, Sunday, Monday 8am - 6pm

Job Description

The Case Manager will provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. The goal is to enhance the quality of patient management and satisfaction while promoting continuity of care and cost-effectiveness through case management, utilization review, and discharge planning.

Responsibilities

Conduct comprehensive assessments of patient needs.

Implement care plans and evaluate their effectiveness.

Coordinate with healthcare teams to ensure continuity of care.

Utilize EPIC for charting and documentation.

Ensure compliance with regulatory and departmental guidelines.

Required Experience / Certifications / Licensure

Minimum of 2 years of healthcare experience as a Registered Nurse.

EPIC experience required.

Licensed to practice as a Registered Nurse in the state of North Carolina.

BLS certification required; ACM/CCM preferred.

No first-time travelers; candidates must live outside a 50-mile radius.
