Requirements & Qualifications:

Job Details:

Schedule: 40 hours/week guaranteed

Patient Load: Average of 6–7 patients per day

Territory Coverage: Up to a 60-mile radius from the hub/office

Types of Cases: Primarily geriatric care

Certifications Required: BLS (American Heart Association)

Experience Required:

Minimum 1 year of recent home health experience

OASIS documentation experience required

EMR experience preferred: HCHB (Homecare Homebase) – online course to be completed prior to assignment

On-Call Requirements:

Every 4th weekend

2–3 weekday after-hours on-call shifts per month

Technology & Equipment:

iPad provided for documentation

Cell phone not provided

Timely communication and documentation required

Dress Code:

Scrubs (any color)

Jeans are not permitted

Travel & Compensation:

Mileage reimbursed at the current IRS standard rate

License:

Active RN license in hand required (Ohio or Compact).
