Pay and Benefits

$40 per hour

$60 per hour overtime

$1246 per week per diem

$500 travel allowance

$2846 per week gross based on 40 hours

Schedule

Full-time, 40 hours per week

Monday to Friday, 8:00 am - 4:30 pm

No weekends or holidays

Occasional additional hours for emergency evaluations

Contract Dates

11/01/2025 - 10/31/2030

Requirements

Degree from an accredited Physical Therapy program

Current state license and Basic Life Support (BLS) certification

Minimum 1 year of Physical Therapy experience in the last 36 months

Proficiency with Electronic Health Records

Proof of required immunizations and recent PPD test

Strong communication skills in English

Must meet all credentialing and background checks

Responsibilities

Evaluate and provide rehabilitation services for patients with a wide range of conditions including post-stroke, arthritis, post-surgical recovery, and chronic pain

Develop and implement patient-centered treatment plans

Provide education and written home exercise programs

Document care in the Electronic Health Record system

Collaborate with physicians and participate in patient-centered medical home model

Maintain professional standards, cultural sensitivity, and patient confidentiality

Participate in committees, staff development, and peer reviews as assigned
