If you’re passionate about making a difference in the lives of patients of all ages, this is the role for you.Your Role As a Home Care Physical Therapist, you will:Evaluate and develop personalized treatment plans for patients.Modify care plans as needed to ensure optimal patient outcomes.Serve as a role model and clinical resource for staff and students.Work flexible hours based on patient needs and program demands.Key ResponsibilitiesDocument patient visits in detail to ensure continuity of care.Educate patients and families on discharge planning and community resources.Update treatment goals promptly and communicate changes effectively.Collaborate with multidisciplinary teams and document interactions.Administer various physical therapy treatments and evaluate patient progress.Conduct comprehensive assessments and develop tailored care plans.Perform wound care and educate caregivers on home exercise programs.Qualifications To thrive in this role, you’ll need:Certifications:Proof of Auto InsuranceBasic Life Support (BLS) certification within 30 days (AHA)Valid Driver’s LicenseLicensed Physical Therapist (PT) in IllinoisEducation: Bachelor’s Degree in Physical Therapy (Required)Working Conditions Enjoy a dynamic work environment where hours may vary based on patient census and program needs.
