Supervised by a Registered Nurse or Physician, the LPN guides our patients through a seamless process to ensure that their care is of the highest quality and delivered in the most efficient manner. Directing the patients through the appointment, provisions of nursing services during care, and phone triage. Collaborates in care planning discussions, based on RN, MD or other Providers -determine urgency of care needs, triage of emergent care situations to the appropriate provider, and scheduling of acute care appointments to maximize patient care opportunities and unit efficiency. Maintains the flow of the Provider schedule during patient care sessions minimizing patient wait time whenever possible. Occasional evening and weekend may be required.

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Range: $25.79-$33.20/hourly

Job Requirements

Qualifications:

1. Education: Fulfills the requirements to be a Licensed Practical Nurse in NY State

2. Experience: Minimum six months of LPN experience preferred. Outpatient primary care and/or pediatric (if applicable) experience preferred.

3. License/Certification Required: New York State licensed practical nurse. Current American Heart Association CPR and AED certification (BLS for Healthcare Providers).

4. Skills:

Knowledge of age-specific medical terminology, anatomy, physiology, and pathophysiology. Familiarity with health care systems, regulations, policies, and functions. Understanding of documentation standards.

Knowledge of equipment, supplies and materials needed for medical treatment. Understanding of basic laboratory procedures including preparation and screening.

Knowledge of infection prevention, infectious disease management and control and safety standards.
