We are seeking a Nurse Practitioner to join our growing team of specialists across our system, specifically atCharlotte Hungerford Hospital, a 122-bed, general acute care community hospital located in Litchfield County and the beautiful Northwest Hills of CT.
Palliative Care Nurse Practitioner (APRN) Role Scope
Enjoy a Nurse Practitioner career within a strong collaborative environment, collaborating inter-team, and across Charlotte Hungerford Hospital, partnering with multiple disciplines including Cardiology, Oncology, Emergency Medicine, Critical Care, and more
Experience work diversity with skills and training in assessment and management of the physical, psychological, emotional, spiritual, and social needs of chronically ill, or end of life patients and their family members
Gain knowledge and experience with a team committed to growth and development of their staff as they provide care managing and prescribing symptom focused medications, including controlled substances
Work within a multidisciplinary team including physicians, RN, social workers, pastoral services
Shift: Monday - Friday

Qualifications
Nurse Practitioner (APRN) Requirements
Graduate of an accredited Nurse Practitioner Masters program preferred
Licensed, or eligible to be licensed, as a Nurse Practitioner (APRN) in the state of Connecticut
National Board Certification as a Nurse Practitioner
Palliative Medicine/Hospice experience preferred
