Job Description

Purpose

The CCBHC Assessment Clinician provides individual, family, and/or group counseling family homes or other community-based locations. The Clinician ensures appropriate screenings and assessments are completed. This position collaboratively develops treatment plans with consumers, families, and collaterals as indicated. The Clinician will also provide crisis management and intervention as appropriate. Regular ratings of progress toward goals and discharge planning are expected. Individual and team-based services will be provided.

Responsibilities Include

· Triage referrals that come in to LUK’s CCBHC

· Participate in the intake/assessment process for CCBHC consumers, meeting all LUK standards and contractual expectations

· Oversee the development and implementation of the treatment plan and progress reviews

· Provide trauma-informed clinical services (individual, family, crisis)

· Complete EBP training(s) and maintain fidelity to identified model(s)

· Maintain timely, accurate, strength-based, and trauma-informed documentation in compliance with agency and funding/regulatory entities

· Collaborate with internal and external members of a multidisciplinary team to support consumers and families in achieving their goals

· Participate in youth- and family-centered treatment team meetings

· Maintain timely and effective communication with consumers, families, and collaterals as indicated in the treatment plan and as needed

· Provide crisis management and intervention as appropriate

· Gain an understanding and commitment to LUK’s Mission, Vision, and Values and provide congruent services

Knowledge and Experience Required

· Master’s Degree in a human services field

· Independent licensure preferred

· Experience and credentials in the delivery of services to individuals with SED, SMI, SUDs/CODs

· Experience implementing evidence-based practices

· Experience working with system-involved families
