Monday - Friday, 4 day-time shifts and 1 late shift required.
Day-time shift: 9:00AM - 5:00PM
Late shift: 11:00AM - 7:00PM

SUMMARY
Family Service League is seeking a full-time Case Manager for our Mental Health Clinic in Huntington. The Case Manager will provide case management support by assisting with applications and intakes. This is not a direct service position.

The Case Manager will provide linkage and/or assist with completion of applications for clinical services, concrete resources including but not limited to medical care, insurance, housing, finances, etc.
Assist with scheduling and follow-up of medication review appointments, psychiatric evaluations, and intakes.
Provide administrative support for clinical services in data tracking and statistics, including but not limited to PSYCKES.
Handle all requests for Medicaid transportation and send monthly rosters for Medicaid recipients who are seen at the clinic.
The Case Manager will assist, as needed, with completion of intake applications for clients who would like to receive treatment at the clinic.
Maintain the tracking of medication-only clients to ensure chart compliance with OMH regulations.
The Case Manager will collaborate with local health clinics to initiate referral and maintain communication in order to improve health outcomes of the behavioral health clients.
Work closely with the clinical treatment team in providing supportive treatment and discharge planning.
Provide additional clerical support to the front desk staff as needed.
All other duties as assigned.
QUALIFICATIONS
High School diploma or equivalent required.
A minimum of 1 year of related experience in working with a behavioral health population.
Previous case management experience is a plus.
Excellent interpersonal, verbal, and written communication skills required.
Strong judgement and problem-solving skills required.
Ability to multi-task and prioritize in a constantly changing, fast paced environment.
Proficient computer skills, including Microsoft Office and Microsoft Teams, required.
Bilingual in Spanish preferred.
PHYSICAL REQUIREMENTS
This position requires sitting for long periods of time.
