Key Responsibilities

As a Primary Care Physician (PCP) and leader of the care team, you will provide comprehensive primary healthcare to a dedicated patient panel while ensuring exceptional, value-based care to the community.

Deliver patient-centered, comprehensive primary care services aligned with a value-based care model

Actively participate in quality improvement programs and contribute to practice performance metrics

Maintain compliance with privacy, regulatory, and quality standards, ensuring adherence to HealthyU Family Medicine policies, procedures, and protocols

Implement and support innovative healthcare technologies, services, and programs to enhance community health and wellness

Collaborate with clinical and administrative teams to ensure high-quality patient care

Perform other duties as assignedRequirements:

Completion of an accredited Medical Program in Family Medicine or Internal Medicine

Valid state-issued medical license (must be active by the first date of employment)

Current BLS (Basic Life Support) certification

Active DEA Registration

Ability to provide two professional references

Two or more years of clinical experience in primary care

Experience providing longitudinal patient care (continuity of care)

Familiarity with managed healthcare systems, including quality assurance, quality improvement, risk management, population health, and disease management programs

Strong ability to work collaboratively within a multidisciplinary team

Excellent communication skills with both patients and colleagues

Self-motivated and results-driven approach to patient care
