Licensed Practical Nurse (LPN) – ACT Team

SeaView Community Services Seward, Alaska

Full-Time $36–$38 per hour

Are you a compassionate and skilled LPN ready to make a meaningful impact in the lives of individuals with serious mental illness? SeaView Community Services is seeking a dedicated Licensed Practical Nurse (LPN) to join our Assertive Community Treatment (ACT) Team in beautiful Seward, Alaska.

About Us:

SeaView Community Services has been serving the Eastern Kenai Peninsula since 1972, providing essential mental health and substance use treatment services. Our ACT Team is a multidisciplinary, community-based program designed to support individuals with persistent mental health challenges in achieving stability, independence, and a higher quality of life.

Position Summary:

As a key member of the ACT Team, the LPN provides direct nursing care and health education in a mobile, team-based setting. This role involves working closely with clients in the community to provide medication support, health assessments, nutrition education, and coordination of care. You’ll collaborate daily with other team members to ensure integrated, person-centered care.

Key Responsibilities:

Monitor medications and side effects under the direction of the Psychiatrist

Support medication reconciliation and adherence

Provide client health assessments and basic medical support

Deliver nutrition education and support clients with healthy meal planning and grocery shopping

Offer wellness coaching and preventive care education

Maintain accurate documentation in the electronic health record (EHR)

Participate in daily team meetings and community outreach efforts

Support crisis response as needed, alongside ACT team members

Qualifications:

Active Alaska LPN license (or ability to obtain prior to start date)

Experience in behavioral health or community-based care preferred

Strong communication and teamwork skills

Comfortable working in the field and providing services in client homes and community settings

Valid driver’s license and ability to pass background check

Compensation & Benefits:

Hourly Rate: $36–$38, DOE

Full-time, 40 hours/week
