Part Time Care Partner / CNA (formerly STNA) for South Franklin Circle in Chagrin Falls Ohio

Shift is 5PM to 9PM Monday, Wednesday, and Friday. 12 Hours per week

Non-Union

Position Summary:

The Care Partner is a member of a self-managed work team and is responsible for successfully coordinating the team's efforts to provide the best care possible to the members of the Crane House. The responsibilities of the Care Partners include providing personal care, cooking, housekeeping, as well as planning and leading activities meaningful to the members all while maintaining the members' independence by promoting autonomy and choice in their daily activities. The care partner will build a relationship with every member based on trust and respect and all care performed will be done in such way to uphold the members' dignity, to create a supportive environment of true home and community, and to add to the enrichment of each member's life.

Essential Duties and Responsibilities:

Get to know each individual member's preferred routine and unique wants and needs.

Create choice and promote autonomy of members in all aspects of personal care.

Assist in creating a chart of each member's personal history and interests and utilize these charts to create a quality plan of care for each member.

Carefully follow the plan of care for each member created by the care giving team consisting of the member, family, care partners, clinical partners and the Coordinator.

Ensure that the line between providing personal care and providing clinical treatment is never crossed.

Assist member with personal care functions, including but not limited to:

Dental and mouth care

Bathing

Dressing and grooming

Hair care

Nail care

Shaving

Bowel and bladder care

Other personal and hygiene care

Assist in moving, positioning, and transporting members into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.

Observe, monitor, and report all significant changes in physical, emotional, and cognitive wellbeing of members.

Meet with the Clinical Partners as scheduled or as needed to discuss members' needs and progress and to identify and correct potential problems.

Report all accidents and incidents to the Coordinator or Clinical Partner when they occur.

Report any resident abuse or suspected abuse to the Coordinator or Clinical Partner immediately.

Ensure call system is working at all times and answered promptly.

Communicate with co-workers at all levels to adequately meet the needs of members.

Attend and participate in all team meetings to continuously improve work processes, to share information, and to improve person-centered care provided to all members.

Coordinate with care partners to accomplish the team goal of providing the best care possible to the members.

Attend and participate in orientation programs, ongoing training, and educational classes.

Follow established safety procedures and observe, monitor, and intervene in or report to the Coordinator unsafe conditions in the Crane House.

Maintain confidentiality and privacy regarding residents' health and personal information.

Clean members' apartments and ensure everything is working properly.

Plan and provide meaningful individual and group activities based on the members' preferences.

Prepare and serve meals in an attractive, clean, and timely manner.

Clean up after meals and snacks which will be planned according to the needs and preference of members.

Keep kitchen and pantry areas clean and ensure all appliances are working properly.

Ensure cleanliness of the dining room by checking between meals and conducting necessary spot cleaning.

Ensure all common areas including hallway, library, social room, and lobby are clean and organized throughout the day.

Launder clothing and other personal items as requested by each member.

Perform all other related duties as needed by the members of the Crane House. Behavioral competencies:

Enjoys all aspects of caring for older adults with the goal of maintaining the person's independence and keeping the care specific to each member.

Takes pleasure in socializing with older adults whether it's during a planned activity or a spontaneous conversation throughout the day.

Conveys a sense of understanding and regard for other people's feelings, especially with individuals who have physical or cognitive impairments.

Exemplifies the highest level of customer service. For example: greets members and visitors with a smile and positive attitude; calls members by Mr. or Mrs.; uses a member's first name only if it has been requested; knocks on a member's door and waits for a response before entering; goes above and beyond to respond to "spur of the moment" member requests; and takes ownership of all observed problems regardless of where the problem has occurred.

Provides honest and open communication with members, families, and coworkers.

Works as a team player within the Crane House and all other departments.

Willing to pitch in to complete a task even when it's outside of the job description.

Attention to quality and detail of work such as in the neatness and cleanliness of the members' living area or the presentation of the meals.

Highest regard for personal appearance: uniform, hair, and nails are neat and clean.

Strong communication, oral, written, and listening skills. Minimum Qualifications, Education and Certifications:

High school diploma or GED.

Training and/or experience in caring for the elderly.

Experience as a nursing assistant, home health aide, recreational therapist, dietary worker, or housekeeper in a senior living setting is a plus.

Physical demands may include walking, talking, hearing, kneeling, crouching, stooping, reaching, pushing, climbing a ladder, manipulating, standing for extended periods, lifting up to 75 pounds, transferring resident up to 150 pounds with assistance, and repetitive motion.
