Description
GENERAL SUMMARY:
Provides diagnostic and rehabilitative services to patients with speech language and/or cognitive disorders. These are provided under the direction of the physician, with the participation of other health team members, family, and significant others as needed.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Assists the physician in evaluating and diagnosing speech and language disorders and level of functioning by evaluating causes and test results.
Participates in the development of the Plan of Care.
Interprets findings to the physician, patient, family, health team members, and significant others.
Provides comprehensive speech therapy in the patient's residence within the guidelines of the American Speech, Language and Hearing Association (ASHA).
Demonstrates, teaches and supervises communication strategies to the patient, family, health care team and significant others to maintain/improve skills.
Advises and consults with patient, family and other health professionals in implementing and evaluating the Plan of Care.
Revises the Plan of Care as indicated, upgrades program based on the patient's progress and participates in discharge planning.
Provides supportive care to the patient, family and significant others and assists them to accept and adjust to realistic plans and goals on the patient's physical limitations.
Provides interdisciplinary communication to insure the effective implementation of comprehensive treatments.
Prepares clinical and progress notes and completes required documentation promptly, accurately and concisely in accordance with Agency Policy, Medicare, and other third party reimbursement regulations.
Has access to electronic or manual protected health information by scope and responsibility. Will be required to adhere to the privacy practices as detailed in the Notice of Privacy Practices, privacy policies and procedures.
Maintains Professional License by completing the State mandated Child Abuse CEU’s.
OTHER DUTIES AND RESPONSIBILITIES:
Maintains confidentiality of patient Information.
Participates in periodic patient evaluation conferences and Patient Chart Review Audits, as needed.
Identifies the need for medical intervention and for interdisciplinary referrals and acts accordingly to see that those needs are met.
Assumes other duties as assigned by management.
Qualifications

KNOWLEDGE, EXPERIENCE, LICENSURE REQUIRED:
Masters Degree in Speech Pathology.
Certified Clinical Competence (CCC) in Speech Pathology, certified by the American Speech and Hearing Association (ASHA) or eligibility for CCC (e.g. Clinical Fellowship year).
Minimum of one (1) year experience as a Speech Language Pathologist under the supervision of a Certified Speech Pathologist.
Comprehensive knowledge of speech language disorders, theory and therapy techniques and the ability to adapt them to home situations.
Knowledge of current home health insurance reimbursement regulations.
Currently licensed to operate a motor vehicle in Pennsylvania and necessary insurance coverage as mandated by the State.
Maintains current CPR certification.
PHYSICAL JOB DEMANDS:
Ability to physically move, lift or shift 100 lbs.
Ability to stand, walk, climb and descend stairs, reach above objects.
Able to safely operate a motor vehicle.
WORKING CONDITIONS:
Normal working hours are Monday through Friday, 8:00 a.m. to 5:00 p.m. or as schedule dictates.
Maintains a safe driving record and a vehicle in dependable working order.
