Corrections Coordinator

The Corrections coordinator is responsible for supporting a positive, collaborative relationship with correctional facilities and other OTP providers to ensure incarcerated individuals receive their recommended services and medication. Major duties will include managing the intakes, coordination of services/transfers, and discharges from correctional facilities for incarcerated clients.

Roles and Responsibilities:

Work closely with referral sources (including but not limited to DOCCS reps and other OTPs) and is primarily responsible for the scheduled and coordination of clients to and from the OTP program, including new admissions, guests, and transfer.

Coordinate new patient admissions, obtaining required documentation, such as screening, intake documentation and medication orders.

Coordinate with counselors, medical staff, and other staff as necessary

Maintain patient charts/records into the EMR system

Schedule assessments, meetings, and consultations

Maintaining billing, filing, copying, faxing, and answer the phone.

Minimum Qualifications:

High school diploma or equivalent. The successful candidate will have an understanding of Microsoft Word and Excel, as well as the ability to prioritize multiple tasks

Desired Qualifications:

AA in human services field with at least one year of experience with care coordination

Skills:

Excellent interpersonal skills

Ability to multi-task, strong attention to detail

Ability to meet deadlines, utilization of EMR software for documentation and communication, and highly organized.

Additional Tasks & Responsibilities:

Promoting excellent customer service

In depth understanding of medical record management related to billing

Demonstrates skills at being an outstanding team member

Proficiency in Microsoft Office applications

Excellent organization skills and ability to work collaboratively with management, support, staff, and community stakeholders

Excellent written and oral skills

Location and Travel Requirements:

Cayuga Addiction Recovery Services - 334 W. State St.

Monday- Friday, on-site at least 3 days a week.

Pay Range Disclosure:

$18.45 to $22 per hour based on experience
