About the Role:

As a Direct Support Professional at HealthChoice Waiver Services, you will play a vital role in empowering individuals with disabilities, developmental challenges, or special needs to lead independent and fulfilling lives. You will provide hands-on support, encouragement, and assistance in daily living activities while fostering a safe, respectful, and engaging environment.

Key Responsibilities:

Assist individuals with daily living tasks, including personal care, hygiene, meal preparation, and mobility support.

Provide emotional support and companionship while promoting independence and self-confidence.

Support individuals in community integration, such as outings, social events, and recreational activities.

Help develop and implement individualized care plans to achieve personal goals and improve quality of life.

Document daily activities, progress, and any concerns to ensure the highest level of care.

Communicate effectively with team members, families, and healthcare professionals.

Qualifications & Skills:

Experience in caregiving, healthcare, or social services is a plus but not requiredtraining is provided!

Compassionate, patient, and dedicated to making a positive impact.

Ability to lift, assist, or provide physical support as needed.

Strong communication and teamwork skills.

Valid drivers license.
