Key Responsibilities:

Collaborate with an interdisciplinary team to create and implement treatment plans based on individual patient needs.

Assist in establishing discharge plans and contingencies to enhance patient experience.

Facilitate weekly team conferences and document all treatment plan modifications.

Maintain adherence to regulations and company policies while monitoring patient care outcomes.

Engage with patients and their families, ensuring clear communication regarding rehabilitation and Case Manager roles.

Conduct assessments and complete documentation within required timeframes.

Participate in utilization reviews and collaborate in solution-oriented discussions.

Qualifications:

Licensure/Certification:

Qualified candidates must hold an active license applicable to their discipline (RN, SW, OT, PT, ST, etc.).

Eligible for CCM® or ACM certification upon entry or within two years in the role.

Minimum Qualifications:

For Nursing: Minimum Associate Degree in Nursing and active RN licensure; BSN preferred.

For other eligible professionals: Minimum bachelor's degree, graduate degree preferred.

Two years of rehabilitation experience is preferred.
