Qualifications as an Advanced Practice *

Board Certified

2+ years of outpatient clinical experience preferred, though not required

Skilled in performing comprehensive and episodic health assessments for a range of acute and chronic conditions

Active, unrestricted license (or eligibility) in the state of Arizona

Active DEA registration (or eligibility)

Enrollment in Medicare/Medicaid (or eligibility)

Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)

Familiarity with Electronic Medical Records
