Be part of something truly special. As a key team member in our PACE program, you’ll help redefine aging by supporting older adults in living with dignity, purpose, and independence. You’ll build trusting relationships, assess individual needs, and coordinate care that honors the whole person. Working alongside an interdisciplinary team, you’ll craft personalized care plans that empower participants to thrive while safely and confidently aging in place.
The Heart of PACE:
At PACE of Southwest Michigan, we believe older adults deserve to age with dignity, support, and connection. As a nonprofit, community-based program, we offer a coordinated system of care that brings together medical, social, and personal services - each one thoughtfully managed by our dedicated interdisciplinary team. With physicians, nurses, social workers, therapists, and more working side by side, we deliver comprehensive, person-centered care that helps Participants remain safe, healthy, and independent while aging in place.
Primary Responsibilities:
Serve as a compassionate advocate for older adults by assessing needs, coordinating services, and helping participants thrive while aging in place.
Collaborate with the Interdisciplinary Team (IDT) on a person-centered non-hierarchical approach (not PCP-driven).
Act independently and take a leadership role in managing participant needs from a social work perspective, with an emphasis on case management and behavioral support.
Work environment is primarily center based with some fieldwork, including home visits to assess participant environments and support care planning as needed.
Connect participants and caregivers to community resources, serving as a liaison and advocate for their needs.
Contribute to a supportive and collaborative team dynamic, engaging daily with nurses, therapists, and other professionals to respond to participant changes and adjust care accordingly.
Focus on social and behavioral aspects of care (rather than traditional clinical therapy), with a strong emphasis on relationship-building and ongoing support.
Embrace the PACE model and bring or develop familiarity with its philosophy and structure.
How you’ll stand out:
Master’s degree in social work from an accredited School of Social Work (MSW).
Current licensure as a Social Worker in the State of Michigan (LMSW or Limited License Social Worker).
Minimum of one year of experience working in a social work setting preferred.
Minimum of one year of experience working with a frail or elderly population (preferred).
Willing to obtain CPR certification upon hire.
