We are looking for a LVN to work the "7 On, 7 Off" Schedule. This person will start Friday at 5 PM, be on call all weekend until Monday at 8 AM. Then, this person will be on call starting Monday at 5 PM until Tuesday at 8 AM, then be off until Tuesday at 5 PM through Wednesday at 8 AM, etc. until that upcoming Friday at 5 PM. At that point, they would have a week straight off.
We need one On Call LVN to support the Eastern areas (Rockwall, Rowlett, Garland, Mesquite, Wylie) as well as one for the Western areas (Fort Worth, Arlington, Grand Prairie, etc.)
Hospice LVN experience is required to be considered. This is a full time, hourly-based role that will be working on call, overnight during the week.
Job responsibilities can include but are not limited to:
Assist the Registered Nurse in carrying out the plan of care.
Assist the Registered Nurse in performing specialized procedures (as defined in the State Nurse Practice Act) as directed by the Registered Nurse under physician's orders.
Assist with the preparation, implementation and continuing evaluation of the patient plan of care.
Assisting the patient with activities of daily living, including the teaching of self-care techniques;
Preparing equipment and supplies for treatment that requires adherence to sterile or aseptic techniques.
Actively participate with the IDG in collaborating to ensure the patients plan of care is kept current and any identified issues are addressed.
Help reinforce teaching to the patient/family members regarding end of life care.
Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences.
Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on the job learning.
Prepare clinical and progress notes as appropriate.
Keep the Case Manager informed of changes noted during visits.
Performs on-call duties (including holidays) per policy and agency need.
Assume responsibility for self-development by continually striving to improve his/her Nursing practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and/or individual research and reading.
Qualifications:
Graduation from an accredited School of Practical Nursing.
Must be licensed in the State of Texas as a Licensed Vocational Nurse
One or more years of nursing experience. Hospice experience required.
Must be a licensed driver with an insured automobile in good working order.
The ability to communicate well in English, both verbally and in writing.
