Valid Ohio driver’s license and proof of auto insurance is required by hospital policy. Must pass a motor vehicle background inspection, insurance eligibility, driving qualifications and training set forth by Nationwide Children’s Hospital and must maintain qualification of insurance guidelines. Job Description Summary:

Provides case management, care coordination, and crisis intervention for children, adolescents, and their families on a crisis community-based team. Maintains the team's patient dashboard, supports clinical interventions, coordinates care activities, and handles the state Data Management System (DMS), all while adhering to professional standards under the supervision of the leadership team.

Essential Functions:

Manages care coordination for a high acuity patient population by placing referrals and ensuring follow-up contact. Supports care coordination activities including scheduling, data entry, report generation, distribution, and communication with internal and external entities.

Maintains knowledge of internal and external resources, including wait times and referral criteria. Reviews incoming referrals in consultation with clinical leadership. Communicates and sends updates to community partners, including care management entities (CMEs). Strives to maintain positive patient satisfaction.

Assists with direct patient care in acute situations, ensuring high-quality care and supporting clinical interventions. Completes clinical charting and other related administrative paperwork in accordance with policy standards and program operating procedures.

Screens for lethality, completes risk assessments, and employs clinically appropriate interventions for crises, such as de-escalation, psychoeducation, skills building, and safety planning, under supervision.

Utilizes EPIC for documenting care coordination activities in compliance with policy standards and collaborates effectively with the interdisciplinary team. Assists with data gathering, entry, and tracking for process improvement, within the MRSS statewide Data Management System (DMS).

Attends in-service trainings, peer reviews, and other interagency and supervisory activities. Meets regularly with the designated supervisor. Participates in routine staff meetings and client treatment rounds.

Education Requirement:

Bachelor’s degree in social work, counseling, psychology, or related field required.

Licensure Requirement:

LSW, required.

Certifications:

Basic Life Support (BLS), required.

First Aid Certified, required.

Demonstrated skills and abilities in providing behavioral healthcare to children, adolescents, and families.

Willingness to seek and accept direct supervision, as well as respect productive feedback from colleagues and others.

Excellent communication and interpersonal skills to establish and maintain positive relationships with patients, families, peers, and community representatives.
