A DAY IN THE LIFE
The Care Management Program Supervisor coordinates and oversees Health Homes, Hospital and Recovery Care Coordination programs. Provides direct supervision, leadership and training to staff to assure that the overall goals and objectives are met. Participates with supervisory team on program management, new program development and outreach activities. A day in the life of a care coordination supervisor involves guiding a team of 8-10 care coordinators that connect clients to health and social services. Meeting with care coordinators individually and in team meetings to review caseloads and staff complex cases. The supervisor also ensures compliance with program and agency standards and policy, provides staff development and training, and acts as a liaison between internal departments as well as community partners to promote integrated, client-centered care.
COMPENSATION & BENEFITS
10-Step pay range: $79,135- $103,259 annually
NWRC salary ranges are comprised of a 10-Step scale. On their employment anniversary, employees are eligible for an annual pay increase to the next step in the associated salary range, subject to satisfactory performance. In addition to the annual pay increases, NWRC assesses cost of living adjustments (COLA) on an annual basis, subject to Governing Board approval. New employees at NWRC usually start at the first step in the positions assigned salary range. NWRC may consider a higher step whenevaluatingthe candidates experience, training, or proven capability. Internal equity will be reviewed and prioritized when determining a starting pay rate.

SCHEDULE & LOCATION
NWRC strives to offer employees schedule flexibility while balancing the needs of their role within the agency with their personal lives. The Care Management Program Supervisor will work with the Executive Director to develop a schedule that includes regular, in person time in the Skagit office as well as optional remote days. Travel to the Bellingham office will be required occasionally based on agency need.
QUALIFICATIONS
Education/Experience:
Masters degree in behavioral and health services (this includes social work, clinical psychology, sociology, guidance counseling, gerontology, nursing) and two years of paid on-the-job social services experience; OR
Bachelors degree in behavioral or health services or a related field AND four years of paid on-the-job social service experience AND;
Two years supervisory experience in an administrative capacity, including supervision of programs and personnel, preferably in a home-visit environment, and overall office management OR
An equivalent combination of education and experience which provides the skills, knowledge, and abilities to perform the work.
Experience working with community-based programs for seniors and people with disabilities preferred.
Knowledge, Skills, and Abilities:
Demonstrated ability to assess client health status, develop appropriate care plans, assess skill and suitability of personal care aides, and provide clinical consultation to care management staff
Knowledge of responsibilities and resources of federal, state and local agencies responsible for administering disability, behavioral health and aging services.
Ability to communicate effectively in both oral and written format.
Ability to work independently, with a minimum of supervision.
Ability to supervise, direct, motivate and train others.
Ability to plan, organize, prioritize and coordinate work assignments and/or projects.
Ability to work as part of a team and provide leadership and structure as appropriate.
Ability to establish and maintain effective working relationships with co-workers, providers and the general public using courtesy, tact and good judgment.
Ability to work with efficiency under frequent pressure of deadlines while maintaining a professional demeanor and to formulate solutions to problems in a timely manner.
Sensitivity to the needs and values that are present in working with diverse groups of people.
Knowledge of computer systems and software programs.
Skill and/or fluency in speaking other languages is desired.
Licenses/Certificates/Special Requirements:
Must have a valid Washington State driver's license, a vehicle available for work-related travel, and appropriate liability insurance.
